Formal and informal assessment will create a picture of where you’re at right now

Together we will set realistic outcomes (goals) and develop a treatment plan to obtain them

We work together, reflecting back on the goals to guide interventions. If your goals change that’s okay! Adapting is a part of life.

Once your established goals are met we’re ready to either set new goals or end service
